Work Environment\n

This position is designed to be fully remote, allowing for flexibility in your work environment while still providing comprehensive support to our community clients. Standard work hours are Monday to Friday, from 9:00 AM to 5:00 PM, enabling a balanced work-life integration.

Key Responsibilities\n

• Serve as a liaison between families and outpatient providers, ensuring that clients receive coordinated care tailored to their individual needs.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively, ensuring proper coverage for our clients.

• Participate actively in discharge planning and aftercare strategies, assessing ongoing needs and connecting clients with appropriate resources.

• Provide therapeutic meal support that emphasizes cultivating a healthy relationship with food, utilizing evidence-based practices to engage clients positively.

• Deliver empathetic, safe, and affirming care that acknowledges and values each client’s intersecting identities, fostering trust and rapport.

Qualifications\n

Compensation and Benefits\n
