This position is primarily remote, allowing for flexibility while working within a detox center dedicated to supporting individuals facing various health challenges. You will collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to provide comprehensive care. While working from home, you will also have occasional visits to clients to ensure personal engagement and ongoing support.

• Conduct comprehensive psychosocial assessments for senior clients to identify emotional, mental, and social needs.

• Develop, implement, and monitor personalized care plans in collaboration with clients and their families, tailored to address specific goals and preferences.

• Facilitate counseling sessions to provide emotional support and help seniors navigate life transitions, including health changes and loss.

• Advocate for clients’ rights and ensure access to essential community resources, enabling them to receive the support they need.
\n
• Engage in regular check-ins and visits with clients, assessing their living conditions and addressing any safety concerns.

• Organize and facilitate family meetings to resolve conflicts and improve communication regarding the care of senior clients.

• Educate clients and families about available resources, support groups, and programs that promote social engagement.

• Keep meticulous and up-to-date documentation in adherence to ethical standards and organizational policies, utilizing electronic health record (EHR) systems.

You will primarily serve seniors who may be coping with chronic health conditions, mental health challenges, or life transitions. Our client population is diverse, requiring a nuanced understanding of aging-related issues. Your role is pivotal in creating a supportive framework that fosters their independence while addressing their unique psychosocial needs.

• Master's degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of professional experience in social work, particularly in elderly care or community settings.

• Strong understanding of social work principles, aging-related issues, and local community resources.

• Excellent communication and interpersonal skills, with the ability to build rapport with clients and families.

• Proficient in using EHR systems, with the capability to maintain accurate documentation.
