This position offers an onsite work setting that involves traveling to various patient homes throughout Elizabeth, New Jersey, providing a dynamic and engaging work experience. Key Responsibilities

• Execute day-to-day operations of providing physical therapy services at patients' homes, ensuring adherence to evidence-based therapeutic practices.
• Conduct thorough initial evaluations and regularly scheduled reassessments, accurately documenting findings in accordance with established guidelines and policies.
• Develop individualized treatment plans based on patient evaluations, incorporating manual therapy modalities, exercise programs, and assistive devices as appropriate.
• Implement treatments effectively to progress patients toward their rehabilitation goals, monitoring progress and adjusting plans as needed to maximize outcomes.
• Collaborate with interdisciplinary teams, including physicians, occupational therapists, and social workers, to coordinate comprehensive care for patients.
• Educate patients and their families on therapy expectations, injury prevention, and at-home exercises to foster ongoing improvement.

Patient Population / Client Focus

This role primarily engages with a diverse patient population, including older adults, post-operative patients, and individuals experiencing chronic pain or movement limitations. You will be responsible for adapting your therapeutic approach to accommodate specific age-related considerations and clinical conditions, thereby enhancing patient experiences and satisfaction.

Qualifications

• A Bachelor's Degree in Physical Therapy or related field is required.
• Possession of an active Physical Therapist license in New Jersey with an impeccable record devoid of sanctions or board proceedings.
• Demonstrated expertise in a variety of manual therapy techniques and modalities to provide effective patient care.
• Excellent interpersonal and communication skills, with the ability to effectively teach and motivate patients in a home care environment.

Preferred Experience

Compensation and Benefits
