Key Responsibilities\n

• Conduct thorough assessments of clients’ mental health needs, including interviews, observations, and appropriate screening tools.

• Develop personalized treatment plans that outline the goals and strategies for client progress, ensuring plans are adaptable to ongoing assessments.

• Implement and monitor evidence-based therapeutic interventions, such as cognitive behavioral therapy (CBT) or solution-focused brief therapy, tailored to individual client needs.

• Maintain accurate and confidential clinical records in compliance with HIPAA regulations and organizational policies.
\n
• Collaborate effectively with a multidisciplinary treatment team consisting of other mental health professionals, medical staff, and family members to optimize client care.

• Provide education and support to clients and their families on mental health issues, coping strategies, and available resources.
\n
• Consistently stay informed on the latest research in mental health care and trauma-informed practices to enhance service delivery.

Patient Population / Client Focus\n

Your primary focus will be on clients facing complex emotional and psychological challenges related to their health, including those in hospice care. You will work closely with individuals experiencing grief and loss, ensuring that they and their families receive the necessary emotional support and guidance to navigate this sensitive time.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A valid LCSW license to practice in the state of New York is essential.
\n
• Minimum of 2 years of clinical experience in mental health, preferably in a relevant setting such as hospice care or outpatient services.

• Demonstrated proficiency in implementing solution-focused and trauma-informed psychotherapy interventions.

• Exceptional communication and interpersonal skills, with an ability to build rapport with clients and their families.
