JOB DESCRIPTION TITLE: Housing Intake Coordinator REPORTS TO: Behavioral Health Program Manager / Quality Assurance Manager EMPLOYMENT STATUS: Regular, full-time CLASSIFICATION: Hourly LOCATION: 300 North Willow Street Wasilla AK 99654 FLSA STATUS: Non-Exempt SUPERVISES: Assigned staff GENERAL FUNCTIONS The Housing Intake Coordinator oversees all operations of the My House recovery housing, sets housing guidelines, trains staff working within the houses and oversees occupants, eligibility and operations. Additionally, the Housing Intake Coordinator collaborates with the Quality Assurance Manager to ensure accurate data entry and compliance with intake requirements. ESSENTIAL DUTIES AND RESPONSIBILITIES Conduct intake assessments with individuals referred to the My House behavioral health program to gather relevant background information, assess program eligibility, identify service needs, and explain program guidelines, expectations, and available supports. Receive, review, and respond to referrals for My House behavioral health programs and housing services in a timely and professional manner. Coordinate the intake process from initial referral through admission, including communication with referral sources, clients, residents, and appropriate My House staff to support a smooth transition into services. Coordinate and complete all admission and discharge procedures, ensuring required documentation, notifications, and follow-up activities are completed accurately and timely. Open and maintain client charts in the appropriate electronic health record or database system, ensuring required demographic, referral, intake, service, and discharge information is entered promptly and accurately. Complete all required intake paperwork, discharge GPRAs, and other program documentation, and enter required information into the OPUS database and other applicable systems in accordance with program requirements. Ensure intake, service, and discharge documentation is complete, accurate, confidential, and maintained in accordance with My House policies, funding requirements, behavioral health standards, and applicable regulatory requirements. Comply with all guidelines, procedures, documentation standards, and regulations established by applicable regulatory agencies and funding sources. Maintain consistent communication with referral sources and community partners regarding referrals, intake scheduling, program availability, client needs, and the status of the intake process. Communicate effectively and work collaboratively with behavioral health professionals and other team members to support clients in establishing and accomplishing goals, developing healthy boundaries, accessing services, and working toward increased independence and stability. Support client engagement and retention by building respectful, professional relationships with clients and helping connect them with appropriate My House programs, activities, behavioral health services, and community resources. Assist with client outreach and engagement efforts, including communicating with current and prospective clients regarding My House programs, activities, services, and available resources. Assist with the development and distribution of the My House newsletter, including gathering program updates, client activities, community resources, and other relevant information. Coordinate the My House Supper Club, including scheduling, communication, outreach, and coordination with staff, clients, volunteers, and community partners as needed. Coordinate and support volunteers, including communication, scheduling, orientation, and coordination of volunteer activities with My House programs and events. Maintain a professional, respectful, and trauma-informed approach when interacting with clients, community members, referral sources, volunteers, and community partners. Use appropriate language, maintain client confidentiality, and provide positive feedback that encourages constructive growth and supports the mission of My House. Perform other duties and tasks as assigned or negotiated. MINIMUM REQUIREMENTS High school diploma or general equivalency degree If in recovery must be in remission for 6 months and working a program of recovery. CPR/First Aid/AED certified or able to obtain within 90 days of hire. Must have a valid Alaska Driver's License and insurable under My House vehicle Insurance. PREFERRED REQUIREMENTS Bachelor’s degree in social work, Psychology, Human Services, Counseling, or Substance Abuse/Addiction Counseling Relevant experience may substitute for educational requirement on a year for year basis Alaska Chemical Dependency Counselor or Peer Certification I (CDC I or PSP I) working towards a (CDC II or PSP I) in accordance with State requirements Working knowledge of National Association of Recovery Residence (NARR I or II) Current CPR/First Aid/AED certified Working knowledge of AKAIMS database A developed understanding of DAP notes and billable services SKILLS/COMPETENCIES Demonstrate the ability to respond to crisis situations and emergencies Working knowledge of housing resources, rapid rehousing, permanent supportive housing and transitional housing. Ability to maintain healthy boundaries with clients Demonstrate flexibility, adaptability, and initiative Working knowledge of health care portability act (42CFR and HIPPA) Demonstrate positive work ethics Ability to communicate proficiently verbally and in writing POSITION SCHEDULE This is a full time, hourly non-exempt position. The required work schedule is Saturday - Wednesday, 9AM to 5PM.
