Responsibilities
- Evaluate and treat patients in an outpatient/home health setting, developing and implementing individualized physical therapy plans of care
- Conduct initial assessments, establish goals, and document progress toward measurable outcomes
- Administer therapeutic exercises, manual therapy, gait training, modalities, and assistive devices as appropriate
- Reassess patient status regularly, adjust treatment plans, and communicate changes to the care team
- Collaborate with physicians, nurses, caregivers, and family members to support safe, effective therapy at home
- Maintain accurate, timely documentation and ensure compliance with all regulatory and facility policies

Requirements
- California Physical Therapy license (active, in good standing)
- BLS certification required
- 1 year of experience in the submitted unit specialty within the last 3 months; 2 years preferred
- Experience in outpatient/home health settings and ability to work in the listed assignment setting
- Ability to travel for a 13-week travel assignment and commit to the stated shift schedule
