Key Responsibilities\n

• Conduct ongoing assessments to determine the needs of patients, tailoring your approach to each individual's unique circumstances and challenges.

• Maintain effective communication with supervisors, direct care staff, families, and guardians to provide holistic support for addressing each client's progress.

• Develop and provide a structured schedule to supervisors for appointments and home visits, ensuring careful monitoring of patient engagement and outcomes.

• Coordinate home visits for observations, data analysis, and chart reviews, adapting procedures as necessary to meet program goals.

• Implement the program plan robustly and effectively, ensuring all aspects are carried out as designed to maximize client benefits.

• Actively participate in Interdisciplinary Team Meetings to discuss client updates, share insights, and collaboratively strategize on treatment methods.

• Document monthly and quarterly progress notes, capturing significant developments and changes in clients' mental health status.

• Organize and lead training sessions to ensure that staff members are equipped with the necessary skills to implement program objectives and meet documentation standards.

• Conduct follow-up interviews and home visits as required, providing continuous support tailored to the evolving needs of clients.

Qualifications\n
