Position Overview

Empower Concierge Rehab and PT is actively seeking a dedicated and certified Outpatient Physical Therapist to join our team at our state-of-the-art rehabilitation center located in the picturesque Delray Beach, Florida. In this full-time position, you will be instrumental in delivering high-quality care to our patients. Your primary responsibilities will include evaluating patient needs, devising personalized treatment plans, and facilitating their progress towards recovery and improved quality of life.

Work Environment

Your work setting will be an onsite rehabilitation center equipped with the latest therapeutic technologies and rehabilitation tools. Our clinic operates Monday to Friday from 9 AM to 5 PM, allowing for a balanced work schedule. Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical capabilities, limitations, and rehabilitation needs, establishing achievable and personalized goals.

• Develop and implement individualized treatment programs using evidence-based practices tailored to each patient's unique condition, whether they are recovering from surgery, managing chronic pain, or dealing with sports-related injuries.

• Educate patients and their families on rehabilitation techniques, proper exercise regimens, and lifestyle adjustments to facilitate optimal recovery and prevent re-injury.

• Collaborate with referring physicians and other healthcare professionals, including Physical Therapy Assistants (PTAs), to ensure comprehensive patient care and integrated treatment approaches.

• Maintain accurate and timely documentation of patient progress, treatment modifications, and outcome measures in compliance with regulatory standards and organizational protocols.

• Foster a positive, encouraging environment that promotes motivation and engagement among patients throughout their rehabilitation journey.

Patient Population / Client Focus

At Empower Concierge Rehab and PT, you will work with a diverse patient population ranging from post-operative recovery patients to athletes recovering from sports injuries, as well as individuals managing chronic pain conditions. Each patient's journey is unique, and your role will be vital in customizing treatment programs that resonate with their specific needs and goals.

Qualifications

• A valid Physical Therapist license in the state of Florida (or eligibility for licensure).

• A bachelor's degree in Physical Therapy from an accredited program; candidates with a Doctor of Physical Therapy (DPT) will be favored.

• Strong communication, organizational, and interpersonal skills to effectively engage with patients, families, and multidisciplinary teams.

• A demonstrated passion for patient care and enthusiasm for ongoing professional development.

Preferred Experience

• A minimum of 2 years of experience in an outpatient clinical setting is preferred. However, new graduates are encouraged to apply, as mentorship and training will be provided.

• Experience with specific populations (e.g., orthopedic, geriatric) is a plus, enhancing your ability to cater to a broader range of patient conditions.
