Key Responsibilities

• Conduct thorough evaluations of patients in home care settings to determine their needs and set appropriate treatment goals.
• Develop and implement effective treatment plans encompassing diverse therapeutic methods, such as targeted exercises, manual therapy techniques, and the use of assistive equipment.
• Document and track patient progress meticulously, ensuring all treatments and responses are accurately recorded for effective continuity of care.
• Exercise ongoing assessment by reevaluating patients periodically, adjusting therapeutic goals and treatment plans based on their recovery progress.
• Guide and train patients and their families on the correct usage of assistive modalities, fostering independence in daily living activities.
• Stay up-to-date with the latest advancements in physical therapy through continued education and participation in relevant professional networks.

Patient Population / Client Focus

Your role will primarily focus on helping individuals who are recovering from injuries, surgeries, or chronic conditions in a home care environment. You will work closely with various health team members to ensure that the needs of both pediatric and adult patients are being met holistically. Personal interactions with patients and caregivers will enable you to provide compassionate care tailored to each individual's circumstances.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is essential.
• Current state licensure in Physical Therapy is required to practice in New Jersey.
• Excellent verbal and written communication skills to effectively interact with patients, families, and multidisciplinary team members.
• Proficiency in word processing and familiarity with electronic health record systems for documentation purposes.

Compensation and Benefits
