Key Responsibilities\n

• Conduct comprehensive assessments to evaluate patients’ needs, ensuring that individualized care plans are developed and implemented effectively.

• Prepare and share a tentative schedule detailing patient appointments, home visits, and monitoring activities to enhance therapeutic outcomes.

• Coordinate in-home observations, chart reviews, staff interviews, and data analysis, ensuring compliance with all care protocols and enhancing service delivery.

• Ensure that treatment plans are followed accurately, reviewing progress and modifying approaches as needed to align with evolving patient needs.

• Participate actively in interdisciplinary team meetings, engaging collaboratively with colleagues to assess and improve treatment strategies.

• Document and complete timely monthly and quarterly progress notes, maintaining transparency and accountability in patient care.

• Organize and conduct training sessions for staff regarding the implementation of treatment protocols and documentation practices.

• Carry out follow-up interviews and home visits to extend continued support for the individuals you serve, reinforcing progress and engagement.
