Key Responsibilities\n

• Conduct thorough psychosocial assessments utilizing interviews, clinical observations, and reviews of patients’ medical records to evaluate various factors affecting their recovery process.

• Provide individual and group counseling to patients and their families, focusing on emotional support, coping strategies, and adjustment issues related to life changes.

• Collaborate closely with rehabilitation teams, including physicians and therapists, to develop and implement comprehensive, individualized treatment plans that address both psychosocial and physical health needs.

• Facilitate access to vital community resources, such as vocational rehabilitation programs, housing assistance, and support groups, ensuring a continuum of care that extends beyond our facility.

• Educate patients and families about available community resources and services post-rehabilitation to aid in successful transitions back into daily life.

• Play an integral role in discharge planning, ensuring that patients are fully prepared for their transition home or to other care environments.

Patient Population / Client Focus\n

Our primary focus is on individuals in rehabilitation, ranging from those recovering from acute medical events like strokes to those with chronic conditions requiring long-term management. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required for this role.

• An active license as a Licensed Clinical Social Worker (LCSW) in the state of New York.
