Key Responsibilities\n- \n
- Ensure timely and thorough confirmation of discharge appointments for patients and their families, facilitating a smooth transition from care. \n
- Communicate effectively with referral sources and outpatient providers to ensure continuity of care during the patient’s admission, midpoint, and discharge. \n
- Lead therapy sessions, offer milieu treatments, and participate in psychoeducational and process-oriented therapy groups, addressing the unique psychological needs of patients. \n
- Conduct clinical evaluations and crisis management during sessions, addressing patient needs with sensitivity and expertise. \n
- Facilitate patient transfers to higher levels of care when necessary, ensuring seamless service delivery. \n
- Engage in clinical outreach and provide program tours, assisting in expanding our reach and community awareness. \n
Our LCSW will primarily work with terminally ill patients in hospice, focusing on those facing complex emotional and psychosocial challenges. This role is designed for social workers who are skilled in addressing issues related to grief, loss, and end-of-life decisions, ensuring that patients and their families receive both emotional support and practical assistance as they navigate this difficult time.

- A Master’s degree in Social Work from an accredited institution. \n
- Current and active licensure as a Clinical Social Worker (LCSW) in California. \n
- Strong written and verbal communication skills, with the ability to convey complex information clearly and compassionately. \n
- Capability to establish and maintain appropriate professional boundaries with clients, demonstrating respect and integrity. \n
- Experience with timely documentation of services rendered, ensuring compliance with organizational standards and regulatory requirements. \n

- Prior experience in hospice, palliative care, or similar healthcare settings is strongly preferred. \n
- Experience leading therapeutic groups or sessions aimed at addressing end-of-life issues and facilitating difficult conversations. \n

- Expertise in crisis intervention and de-escalation techniques. \n
The annual salary for this position ranges from USD 120,000 to 130,000, commensurate with experience and caseload management.
