Key Responsibilities\n

In your role as an LCSW, you will be central to our patients' psychological well-being. Your daily responsibilities will include:

• Conducting in-depth clinical assessments to gather comprehensive patient histories and inform treatment planning.

• Designing and implementing individualized treatment plans that utilize evidence-based therapeutic interventions, such as Cognitive-Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).

• Maintaining accurate and up-to-date clinical documentation, ensuring compliance with HIPAA regulations and organizational standards.

• Interacting and collaborating effectively with a multidisciplinary team of healthcare providers, including physicians, nurses, and other social workers, to ensure coordinated care.

• Participating in ongoing case consultations and peer review sessions to promote best practices and improve service delivery.

• Educating patients and families about mental health conditions and available treatment options, while also connecting them with valuable community resources.

• Staying informed of the latest research in mental health and incorporating new knowledge into your therapeutic practices for improved patient outcomes.

Your work will primarily focus on supporting patients facing terminal conditions and their families, providing emotional support during a challenging time. Understanding the unique needs of this population is crucial, and your expertise will help guide them through their emotional journey with dignity and respect.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is a must.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• A minimum of two years of post-licensure experience providing clinical social work services, particularly in hospice or palliative care settings.

• Proficiency in conducting diagnostic assessments and creating effective treatment plans tailored to individual client needs.

• Strong interpersonal and communication skills, with demonstrated ability to establish rapport and trust with a diverse range of patients and family members.

Compensation and Benefits\n
