About the Role We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Responsibilities Provide evidence-based, patient-centered care to residents in skilled nursing facilities Conduct on-site visits and remote patient monitoring to detect abnormalities and prevent hospitalizations Collaborate with care teams to develop and implement individualized care plans Maintain accurate and timely documentation of patient care and services Participate in quality improvement initiatives to enhance patient outcomes and facility ratings Requirements Active NP or PA license (in the state of practice) 1+ year of experience in Geriatric care and Documentation review Excellent communication, collaboration, and teamwork skills Ability to work independently and as part of a remote team What We Offer Annual salary range of $23000 - $25000 Hybrid role with flexibility to make your own schedule Support and training from our clinical directors, regional managers, and supervisors Generous paid time off Health Insurance for full-time employees Free CEUs Referral Bonuses #J-18808-Ljbffr
