Key Responsibilities

• Provide high-quality, patient-centered care by managing the full spectrum of therapeutic processes for a dedicated group of patients, in close conjunction with physicians and the care team.
• Conduct thorough patient assessments and develop customized care plans that prioritize patient engagement, goal achievement, and positive health outcomes.
• Deliver therapeutic interventions and manage the continuum of care for patients, ensuring that treatment aligns with physicians' plans and leverages evidence-based practices.
• Utilize professional assessment skills to evaluate therapeutic, rehabilitative, and functional statuses, subsequently contributing to the comprehensive care plan for each patient.
• Administer skilled therapeutic techniques at the patients' homes, tailoring approaches to yield significant improvements in their physical function as directed by medical guidelines.
• Educate patients, family members, and caregivers on effective therapeutic techniques and the use of supportive devices, empowering them to continue care outside of therapy sessions.
• Coordinate and prioritize your caseload effectively, determining daily staffing needs while managing case hand-offs and team assignments.

Patient Population / Client Focus

Qualifications

• Possession of a Doctor of Physical Therapy (DPT) degree, demonstrating your advanced training and commitment to professional excellence.

Compensation and Benefits
