Key Responsibilities\n

• Conduct thorough assessments of client needs, creating personalized care plans to guide interventions.

• Maintain clear and open communication with supervisors, direct care staff, family members, and other stakeholders regarding client progress and treatment strategies.

• Monitor progress through regular documentation, including monthly and quarterly notes, to ensure compliance with the established care plan.

• Coordinate home visits and assessments as needed to gather comprehensive insights into the client's environment and support systems.

• Facilitate interdisciplinary team meetings to discuss client cases and collaborate on treatment approaches.

• Organize training sessions for team members to ensure they are proficient in implementing program plans and documentation protocols.

• Provide ongoing support through follow-up interviews and observations, reinforcing the therapeutic relationship and enhancing client engagement.

Qualifications\n
