Key Responsibilities\n

• Work closely with primary care providers to identify, treat, and manage mental health concerns, ensuring a holistic approach to patient wellness.

• Conduct thorough patient intakes which include diagnostic and functional evaluations, followed by informed recommendations for treatment plans to primary care providers.

• Deliver brief, focused interventions for patients experiencing mental health crises or seeking preventative care.
\n
• Evaluate patients’ needs and suggest appropriate referrals to specialty mental health services when necessary.
\n
• Follow up on patient progress to assess the effectiveness of treatment interventions and adjust plans as needed.
\n
• Develop and implement relapse prevention strategies for at-risk patients, assisting them in maintaining stable functioning.
\n
• Monitor and coordinate the delivery of behavioral health services, ensuring effective communication and service integration across the care continuum.
\n
• Document patient progress meticulously, maintaining accurate records in treatment charts to uphold compliance with all agency standards.
\n
• Provide educational materials and self-management skills to patients, empowering them to be active participants in their recovery journey.
\n
• Consult on program evaluation and outcome assessment initiatives to improve service delivery and effectiveness.

• Ensure strict adherence to the Code of Ethical Conduct for Clinical Professionals.

• Perform other related duties as assigned to support the operational goals of the clinic.

Patient Population / Client Focus\n

The Licensed Professional Counselor will engage with a wide variety of patients, including children, families, and adults facing a spectrum of mental health issues. Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience.
