Key Responsibilities\n

As an LCSW at Entryway Health and Wellness Clinic, your daily responsibilities will include:

• Conducting comprehensive biopsychosocial assessments to understand clients' needs and develop personalized treatment plans tailored to their goals.

• Implementing evidence-based interventions, such as cognitive behavioral therapy and solution-focused therapy, that directly address the emotional challenges faced by patients and their families.

• Maintaining accurate and confidential clinical records in compliance with ethical and legal standards.

• Collaborating with a multi-disciplinary treatment team to coordinate care and provide a seamless support experience for clients.

• Staying current with the latest mental health research and clinical practices to continually improve service delivery.

Patient Population / Client Focus\n

In this role, you will primarily work with patients at the hospice center who are experiencing end-of-life transitions. Our client population may include individuals with terminal illnesses, those coping with grief or loss, and families seeking support during challenging times. Your interventions will focus on enhancing emotional resilience and offering solace while implementing strategies that promote overall well-being in the face of adversity.

Qualifications\n

The ideal candidate for this position will possess the following:

• A Master's degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Illinois.

• A minimum of two years of clinical experience in a social work capacity.
