Key Responsibilities\n

• Quickly establish rapport with patients, their families, and team members by employing active listening and empathetic communication, ensuring information is conveyed clearly and respectfully.

• Participate as an essential member of a multidisciplinary team, coordinating with primary care physicians and other providers to ensure comprehensive care delivery.

• Conduct thorough screenings utilizing evidence-based methodologies for a range of mental health conditions including depression, anxiety, and substance abuse, followed by appropriate referrals for further assessment when necessary.

• Develop and manage a patient caseload, providing follow-up interventions and proactive care management.
\n
• Utilize electronic medical records to document patient encounters accurately within 48 hours to support seamless integrated healthcare practices.

• Create and implement individualized care plans, working closely with patients, families, and other stakeholders to align services with the identified health needs of patients.
\n
• Maintain strict confidentiality and safeguard patient privacy in accordance with HIPAA regulations.
\n
• Apply principles of population-based care to enhance outcomes for both preventive and chronic health conditions.

• Engagement in supervision sessions, case conferences, and quality improvement initiatives to foster personal growth and enhance service quality.

• Facilitate referrals to psychiatric professionals when medications are prescribed, ensuring comprehensive care.

• Return patient and family calls promptly, addressing concerns and booking appointments in a timely manner.

• Identify and connect patients with relevant community resources, including educational programs, vocational training, and support groups, to augment their health and social well-being.

Qualifications\n
