Work Environment\n

This position allows for a flexible remote work setting while servicing clients at our Community Health Clinic. Expect to engage in virtual consultations and in-person visits when necessary to evaluate client needs. The work hours are typically Monday to Friday, from 8:00 AM to 4:30 PM, with the possibility of evening and weekend hours as needed to accommodate client schedules. On-call availability may be required to support urgent client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, addressing emotional, mental, and social factors influencing their well-being.

• Maintain meticulous documentation of clients' assessments, care plans, and progress in compliance with ethical and legal standards.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.
