Community Health Navigator & Care Coordinator

City of Lubbock is seeking a Community Health Worker to coordinate care with medical providers, social services, and community resources. The role involves education, in-home and community outreach, and working across clinical and community settings. Responsibilities include documenting encounters, developing care plans, and facilitating access to housing, transportation, and health services.
