Key Responsibilities\n

As a Senior LCSW, your primary responsibilities will include:

• Conducting thorough psychosocial assessments for clients entering hospice care to understand their emotional, social, and spiritual needs.

• Developing and implementing individualized care plans in collaboration with an interdisciplinary team, including physicians, nurses, and chaplains, to optimize client outcomes.

• Providing tailored counseling and support services to clients and their families, helping them navigate emotional challenges such as grief, anxiety, and depression.

• Utilizing teletherapy platforms to deliver therapeutic interventions and maintain regular follow-ups, ensuring continuity of care throughout the end-of-life journey.

• Staying current with best practices in hospice care and mental health by participating in professional development, quality improvement initiatives, and relevant training sessions.

• Documenting all client interactions meticulously in our Electronic Health Record (EHR) system, ensuring compliance with regulatory standards.

Patient Population / Client Focus\n

In this role, you will primarily support clients who are nearing the end of life, along with their families who may be facing emotional and practical challenges. You will play a pivotal role in addressing the unique psychological needs associated with terminal illnesses, providing grief counseling, facilitating family communication, and aiding in advance care planning to ensure that each client receives dignified care aligned with their personal values and wishes.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master's degree in Social Work from an accredited program.

• Currently licensed as a Licensed Clinical Social Worker (LCSW) in Virginia.

• Proficiency in conducting clinical assessments, interventions, and treatment planning tailored to diverse client populations.

• Strong communication skills with the ability to collaboratively work within a multidisciplinary team.

• Experience with electronic health record (EHR) systems and telehealth platforms is preferred.

Preferred Experience\n

We require at least 2 years of post-master's practice in social work, with a focus on hospice or palliative care settings preferred. Additional certifications in grief counseling, trauma-informed care, or related areas are advantageous and will be considered favorably.

Compensation and Benefits\n
