Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through detailed interactions, observations, and a thorough review of patient's medical history to understand emotional, social, and environmental influences on recovery.

• Provide empathetic counseling and emotional support to patients and their families, addressing issues related to adjustment, grief, and coping with end-of-life transitions.

• Collaborate with an interdisciplinary team, including doctors, nurses, and therapists, to formulate and implement tailored treatment plans that address psychosocial needs.

• Guide patients and their families in accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance.

• Engage in educational outreach for patients and families, offering insights about available community resources and post-rehabilitation services.

• Participate actively in discharge planning discussions, aiding transitions for patients returning home or moving to alternative care settings.

Qualifications\n
