Key Responsibilities\n

• Conduct initial patient intakes, focusing on thorough diagnostic and functional evaluations, and developing personalized treatment recommendations for the primary care team.

• Provide brief, targeted interventions for patients requiring mental health services, effectively addressing immediate concerns while laying the foundation for ongoing support.

• Work collaboratively with primary care providers to identify patients in need of referral to specialty mental health facilities, ensuring they receive comprehensive support in accordance with their individual needs.

• Develop and implement relapse prevention strategies, assisting patients in maintaining their mental health stability and monitoring their progress through follow-up communications.

• Document and track patient progress meticulously in the treatment chart, ensuring all information is up-to-date and communication with primary care providers is seamless.

• Educate patients in self-management skills and assist them in navigating their recovery journey, preparing them for active participation in their treatment plans.

• Offer consultation to clinic management and fellow team members on behavioral health services, contributing to program evaluation and outcome assessment.

• Assist patients in compliance with medical treatments initiated by primary care providers, including strategies to effectively manage potential side effects of medications.

Qualifications\n
