Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions by utilizing interviews, observations, and physical assessments to identify their capabilities and limitations.
• Develop customized therapy treatment plans based on individual patient assessments, taking into account their personal goals, lifestyle, and preferences.
• Implement evidence-based manual therapy techniques paired with therapeutic exercises aimed at improving mobility, strength, and overall physical function.
• Educate patients and their families on effective strategies for pain management, recovery techniques, and necessary exercises to perform outside therapy sessions.
• Document all evaluations, treatment sessions, progress notes, and outcomes diligently, maintaining compliance with regulatory guidelines and billing requirements.
• Continuously monitor patient progress, making necessary adjustments to therapy plans, and effectively communicate any relevant changes to both the physician and the patient's family.

Patient Population / Client Focus

Our primary focus includes a diverse patient population ranging from pediatric to geriatric individuals, many of whom are recovering from surgeries, injuries, or managing chronic conditions. Working closely with each patient in their own home allows you to tailor the rehabilitation process to fit their personal environment, enhancing their comfort and compliance. Qualifications

• A Bachelor's Degree in Physical Therapy or higher from an accredited program.
• A valid and active Physical Therapy license issued by the state of New Jersey.
• A minimum of 1-2 years of direct experience working as a Physical Therapist, preferably in home health or outpatient settings.
• Demonstrated proficiency in clinical assessment and intervention techniques along with a thorough understanding of physical rehabilitation methodologies.
• Current CPR and First Aid certification; additional certifications in specialized areas such as geriatrics or neurological rehabilitation are highly desirable.
• A valid driver's license and access to reliable transportation for patient home visits are required.
