Job Title Speech-Language Pathologist, Skilled Nursing Facility and Home Health Location Omaha, NE 68137 Facility GSS NE Omaha Metro Hm Care Address: 12875 Deauville Dr, Omaha, NE 68137, USA Shift & Schedule Shift: Varies; Job Schedule: PRN; Weekly Hours: Varies; Salary Range: $32.50 - $52.00 Job Summary Provide speech therapy services both at a skilled nursing facility and home health. Administer speech and language evaluations, tests, or examinations to diagnose and plan treatments for clients/patients/residents with various speech problems and swallowing disorders. Assess clients/patients/residents with speech, language, cognitive, or swallowing problems and develop individualized treatment plans based on clinical diagnoses. Provide speech and language services, monitor disease progression, and adjust treatments accordingly. Instruct clients/patients/residents in practicing speech and language therapeutic exercises; provide consultation for clients/patients/residents working through treatment processes. Ensure all speech and language‑related standards, policies, protocols, and quality requirements are met. May treat individuals of all ages with varying diagnoses and disorders. Demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Follow and adhere to accepted clinical practice guidelines of professional organizations. Demonstrate flexibility with acceptance of variable work schedules/assignments. Participate in activities to promote the department, Good Samaritan Society, and Sanford Health. Interact with individuals outside of the department, including providers, case managers, third‑party payers, public organizations, etc. May supervise assistants, aides, and students as they provide client care, treatment, or other services. Provide ongoing education and training to peers in expert areas as directed by department leadership. Perform other related duties as requested by manager. Focus may include post‑acute, long‑term care, and home health environments when applicable. Qualifications Appropriate education level required in accordance with state licensure. Must be licensed in the state(s) of practice as Speech‑Language Pathologist. When working at a Sanford Health Facility (not required but preferred by GSS Facilities): Holds a Certificate of Clinical Competence issued by the American Speech‑Language Hearing Association (ASHA) or is currently working on a Clinical Fellowship Year (CFY). When applicable, may require a valid driver’s license. Obtain and maintain required department‑specific competencies and certifications. Basic Life Support (BLS) for health care providers is required or must be obtained within the first 60 days of employment.
