Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify emotional, social, and environmental factors affecting patients and their families, particularly in relation to their health, diagnosis, and treatment plans.

• Develop and implement individualized treatment and discharge plans using appropriate social work interventions tailored to each patient's unique situation.

• Collaborate effectively with health care team members, ensuring that patients and families are actively involved in decision-making processes regarding their care.

• Facilitate discharge planning through comprehensive communication with consulting physicians, insurers, and multidisciplinary providers to ensure smooth transitions in care.

• Provide education and counseling to patients and families regarding strategies for adapting to illness and changing life situations, focusing on empowering them during their recovery processes.

• Utilize various therapeutic modalities, including crisis intervention, individual and group therapy, to meet the diverse needs of patients.

• Serve as a resource and mentor for other staff members, fostering a collaborative and supportive team environment.

• Engage in community outreach activities, contributing to educational programs and resources that benefit cancer survivors and their families.

Patient Population / Client Focus\n

Your work will primarily focus on individuals and families impacted by serious illnesses, including but not limited to oncology patients. You will be responsible for assessing and addressing mental health concerns such as depression and anxiety that arise as a result of illness, thereby directly contributing to their emotional well-being and overall recovery trajectory.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current LCSW licensure in the state of California.

Compensation and Benefits\n

The salary range for this position is between USD 70,000 and 100,000 per year, commensurate with experience and qualifications.
