Work Environment

This position is primarily based onsite in the patients' homes, allowing for a personalized approach to care. You will be working independently while also being part of a dynamic interdisciplinary team that includes physicians, occupational therapists, and other healthcare professionals. Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical abilities and rehabilitation needs, using standardized assessment tools.
• Design and implement individualized treatment plans aimed at enhancing strength, mobility, and overall functional independence.
• Administer a range of therapeutic interventions, including manual therapy techniques and prescribed exercises, tailored to address specific physical impairments.
• Monitor patient progress through regular follow-up assessments and adjust treatment protocols as necessary, ensuring that goals are being met.
• Educate patients and their families about effective home exercise programs and strategies for maintaining physical health post-therapy.
• Collaborate with an interdisciplinary care team to develop comprehensive care strategies, ensuring that all aspects of the patient's health are addressed.
• Maintain accurate and up-to-date documentation, complying with state regulations and organizational standards.

Patient Population / Client Focus

This role primarily focuses on serving geriatric patients, many of whom may be recovering from surgery, managing chronic conditions, or experiencing mobility issues. You will be responsible for creating a safe and supportive environment that encourages patients to regain independence and improve their quality of life through effective therapy.

Qualifications

• Current licensure as a Physical Therapist in the state of California.
• A Bachelor's degree in Physical Therapy from an accredited institution; a Master's or Doctorate is strongly preferred.
• Exceptional communication and interpersonal skills, with a strong ability to connect with patients and their families.
• Proficiency in evaluating and documenting patient progress using electronic health record (EHR) systems.
• A commitment to staying informed about advancements in physical therapy practices and ongoing professional development.

Preferred Experience

• Prior experience providing physical therapy in a home care setting or within skilled nursing facilities is highly desirable.
• Familiarity with therapeutic techniques specifically beneficial for the geriatric population.

Compensation and Benefits

We offer a competitive salary ranging from $120,000 to $130,000 annually, complemented by a comprehensive benefits package.
