Work Environment

This position is primarily onsite, as our therapists deliver care in the homes of patients throughout Long Beach. Working in a home care setting allows for a more personalized approach to therapy, where therapists can tailor their interventions to the specific environment and circumstances of the patient. Monday through Friday, you will be part of a supportive team that values flexibility and work-life balance, with hours typically from 9 am to 5 pm.

Key Responsibilities

• Conduct comprehensive evaluations to assess patients' physical capabilities and functional limitations, gathering information from medical histories and physical assessments.
• Create individualized treatment plans that reflect the assessment results and align with the established goals of the patients.
• Perform hands-on therapy techniques, including manual therapy, therapeutic exercises, and modalities such as electrical stimulation, to alleviate pain and enhance mobility.
• Educate patients and their families on effective body mechanics, injury prevention strategies, and proactive health maintenance.
• Monitor and document patient progress meticulously, adjusting treatment plans based on ongoing assessments and patient feedback.
• Collaborate closely with physicians, occupational therapists, and other healthcare professionals to provide multidisciplinary care to our patients.
• Maintain accurate and detailed patient records using our electronic health record (EHR) system, ensuring compliance with regulatory requirements.

Patient Population / Client Focus

Our patient population is diverse, including individuals recovering from surgery, those with chronic pain conditions, and elderly patients requiring rehabilitation to improve mobility and prevent falls. The therapist will work with patients who may have conditions such as osteoarthritis, post-surgical rehabilitation needs, sprains and strains, and gait imbalances. Our focus is on developing therapeutic relationships that encourage active participation in rehabilitation, enhancing the efficacy of our services.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Current state licensure to practice as a Physical Therapist in California.
• Board certification by the American Physical Therapy Association (APTA) is preferred.
• A minimum of one year of clinical experience is required, with preferences for candidates with exposure to a range of orthopedic and neurological conditions.
• Proficiency in advanced physical therapy modalities and techniques.
• Strong analytical, communication, and problem-solving skills to deliver astute clinical assessments.
• Exceptional interpersonal skills to foster relationships with patients and their families.

Preferred Experience

• Five or more years of clinical experience as a Physical Therapist, particularly in home care settings.
• Experience developing and implementing specialized treatment plans for geriatric populations or complex cases.
• Familiarity with electronic health record (EHR) systems and relevant software for patient documentation.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience and qualifications.
