Key Responsibilities\n

• Conduct comprehensive patient intakes, including diagnostic assessments and functional evaluations, to tailor treatment plans in partnership with primary care providers.

• Deliver brief, focused therapeutic interventions for clients requiring mental health support, effectively managing caseloads while adhering to established productivity targets.

• Monitor patient progress closely, initiating follow-ups to assess treatment efficacy and adjusting approaches as necessary.

• Develop and implement personalized relapse prevention plans for patients, fostering long-term stability and improved quality of life.

• Provide education and support to patients, encouraging self-management skills that empower them in their own recovery journey.

• Work collaboratively within an interdisciplinary team, ensuring a coordinated approach to patient care and sharing valuable insights regarding behavioral health.

• Maintain accurate and thorough clinical documentation of patient interactions and treatment plans in compliance with all relevant guidelines and agency standards.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field.

• A current and valid LPC (Licensed Professional Counselor) license in the state of Colorado.

• Strong understanding of DSM-IV diagnoses, behavioral therapy methodologies, and family systems.

• Ability to build strong, cooperative relationships with patients, families, and a multidisciplinary health care team.

• Excellent written and verbal communication skills.

Preferred Experience\n

• Bi-lingual proficiency in English and Spanish is desirable but not required.

• Previous experience in a Federally Qualified Health Center (FQHC) setting is a plus.

• Demonstrated expertise in community resource knowledge and utilization.
