Key Responsibilities\n

• Conduct in-depth assessments to comprehend and analyze clients' social, emotional, and environmental circumstances, ensuring personalized service delivery.

• Design tailored service plans that include specific goals, interventions, and methods for monitoring client progress towards achieving their objectives.

• Provide responsive counseling and emotional support to individuals and families facing various challenges, particularly those related to hospice care.

• Collaborate closely with clients to create and refine personalized service plans, actively connecting them to community resources and support networks.

• Deliver crisis intervention as needed, including risk assessments and safety planning, to stabilize clients during times of heightened distress.

• Work in conjunction with community agencies and other organizations to coordinate services, advocate strongly for clients’ needs, and remove barriers to care.

• Engage in community outreach efforts to raise awareness about available hospice and bereavement services and the essential support we offer.

• Actively participate in case conferences, team meetings, and ongoing staff training to enhance collaborative care practices.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is a minimum requirement.

Compensation and Benefits\n
