Key Responsibilities\n

• Conduct comprehensive assessments and evaluations of clients to identify their mental health needs, consider social determinants of health, and set personalized treatment goals.

• Deliver individualized one-on-one counseling to clients experiencing a range of mental health and psychosocial issues, while utilizing evidence-based intervention strategies.

• Facilitate engaging group therapy sessions focused on common concerns among clients such as grief, loss, and coping strategies, enhancing community support and psychoeducation.

• Design and implement tailored treatment plans that incorporate measurable goals and specific evidence-based methods to track client progress effectively.

• Provide timely crisis intervention services including thorough risk assessments, safety planning, and coordination with emergency resources as required.

• Be an advocate for clients’ rights and navigate them through healthcare and community resources to ensure their access to essential services.

• Maintain meticulous and confidential client records, encompassing assessments, treatment plans, progress notes, and desirable outcomes, while adhering to legal and ethical standards.

In this role, you will primarily work with clients within a hospice setting, where the focus is on providing emotional support and counseling to terminally ill patients and their families. Compensation and Benefits\n
