Key Responsibilities\n

• Engage in ongoing communication with physicians and other members of the healthcare team to ensure comprehensive care and to track patient progress and concerns.

• Maintain accurate and timely documentation, including clinical notes and treatment plans, in compliance with departmental policies and best practices.

• Collaborate with healthcare professionals to design and implement effective treatment and discharge plans inspired by thorough psychosocial assessments and evidence-based interventions.

Patient Population / Client Focus\n

You will be working primarily with adults and older adults facing a variety of psychological issues, including chronic illness, grief, depression, and anxiety, often in the context of a hospice care environment. Qualifications\n
