RN Home Care Case Manager Needed. BrightStar Care is Growing in the South Minneapolis Area and looking for an RN Case Manager 4-Days Per Week. Our Case Managers are salaried, work with a team of nurses (both RNs and LPNs) to complete assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. BrightStar Care is a non-Medicare agency, so no OASIS required. Our nursing team is just that, a team and a family of nurses all looking to help keep our clients safe, happy, and healthy in their homes for as long as possible. PTO Accrual from Day 1 and Paid Mileage between clients and office. We expect this RN to have clients primarily in the Minneapolis Metro Area (Bloomington, Edina, Eden Prairie, etc.) Work where you live!

Job Responsibilities:

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
• Collaborate with multidisciplinary team members to optimize patient care and outcomes.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure accurate documentation and compliance with all regulatory guidelines.

Requirements:

• Active RN license in the state of Minnesota.
• Previous experience in a home health or acute care setting is advantageous.
• Exceptional interpersonal and communication skills.
• Committed to delivering high-quality, patient-centered care.
