Key Responsibilities\n

• Engage in collaborative care by working closely with a multidisciplinary health care team to develop individualized treatment and discharge plans based on thorough psychosocial assessments.

• Conduct assessments and implement evidence-based interventions tailored to the unique needs of each patient, while maintaining accurate clinical documentation and progress notes in alignment with clinic policies.

• Provide counseling services and crisis intervention for patients facing emotional and behavioral challenges, employing techniques that enhance their coping mechanisms.

Patient Population / Client Focus\n

Your responsibilities will center around a diverse patient population consisting of individuals and families experiencing terminal illnesses and end-of-life issues. This role requires sensitivity and adaptability to address the complexities associated with grief, loss, and adjustment during this vulnerable life stage.

Qualifications\n

• A minimum of three years of relevant, unsupervised experience post-licensure as a clinical social worker.

• A Master’s degree in Social Work or Counseling from an accredited institution.

• An active and unrestricted LICSW license in the state of Washington.

• Proficient computer skills, including experience with word processing, spreadsheet management, email systems, and electronic medical records.

Compensation and Benefits\n
