Community Outreach Representative - Behavioral Health

Mindify Wellness & Care is seeking an outgoing, professional, and relationship-driven Community Outreach Representative to help expand our presence throughout North Jersey.

This is a primarily in-person, community-based position . The Community Outreach Representative will visit adult day care centers, physician offices, senior centers, healthcare facilities, and community organizations to introduce Mindify’s behavioral health services and develop strong referral relationships.

The ideal candidate enjoys meeting new people, building professional connections, and working independently in the community. This role is an excellent opportunity for someone who wants to help connect individuals and families with compassionate, accessible behavioral healthcare.

Employment Type: Part-time
Schedule: 20–25 hours per week
Availability: Candidates must be available during regular weekday business hours when physician offices, healthcare facilities, and community organizations are open.
Location: Primarily in person and field-based throughout North Jersey

Community Outreach and Business Development

• Conduct primarily in-person outreach throughout North Jersey.
• Visit adult day care centers, physician offices, senior centers, healthcare facilities, and community organizations.
• Introduce Mindify’s behavioral health services and explain how our programs can support their patients, clients, and communities.
• Build professional relationships with administrators, directors of nursing, physicians, office managers, social workers, case managers, and other healthcare professionals.
• Identify potential referral and partnership opportunities.
• Coordinate introductory and partnership meetings with Mindify leadership.
• Represent Mindify at community gatherings, networking events, and relevant industry functions.

Relationship Management

• Conduct regular in-person visits and follow up with prospective and existing referral partners.
• Maintain consistent communication and cultivate long-term professional relationships.
• Collaborate with Mindify’s leadership and clinical teams to understand community needs and support successful referral relationships.
• Serve as a professional and knowledgeable representative of Mindify within the community.

Community Education

• Deliver brief presentations and informational sessions about Mindify’s behavioral health services.
• Educate community partners about available resources, support services, and the referral process.
• Provide appropriate informational and outreach materials during community visits.

Documentation and Reporting

• Accurately document outreach visits, conversations, meetings, referrals, and follow-up activities in the company’s CRM system.
• Submit weekly outreach reports and updates.
• Share community feedback, market insights, potential partnership opportunities, and relevant outreach developments with leadership.

Qualifications

• Excellent communication, interpersonal, and relationship-building skills.
• Outgoing, professional, dependable, and comfortable initiating conversations with new contacts.
• Self-motivated, organized, and able to manage a community-based schedule independently.
• Valid driver’s license and reliable transportation.
• Ability and willingness to travel throughout North Jersey.
• Availability for 20–25 hours per week during regular weekday business hours.
• Experience in healthcare outreach, behavioral health, marketing, community engagement, customer service, business development, or a related field is preferred but not required .
• Bachelor’s degree in public health, healthcare administration, marketing, communications, social services, or a related field is preferred but not required. Relevant professional experience will also be considered.
• Familiarity with North Jersey communities and healthcare organizations is a plus.

Compensation

• $24–$26 per hour , based on experience
• 20–25 hours per week
• Primarily in-person, field-based work throughout North Jersey
