Key Responsibilities\n

• Conduct initial and ongoing assessments of clients to tailor treatment plans that meet their unique needs.

• Engage in collaborative discussions with supervisors, caregivers, and family members to monitor and communicate each client’s progress effectively.

• Provide detailed schedules to supervisors regarding appointments, home visits, and necessary observations for tracking client advancements.

• Facilitate home visits to conduct assessments, chart reviews, and data analysis to glean insights into client circumstances.

• Oversee the implementation of personalized care plans to ensure therapeutic strategies are effective and delivered properly.

• Participate in Interdisciplinary Team Meetings to discuss patient care strategies and contribute to team enhancements.

• Document monthly and quarterly progress notes to maintain comprehensive records of patient engagements and outcomes.
\n
• Coordinate training sessions to ensure all staff members are proficient in the implementation of care plans and documentation standards.

• Conduct follow-up interviews and home visits to offer continued support and adaptation of care strategies as needed.

• Act as a liaison between clients and the broader mental health community, assisting in the development of support services.

Qualifications\n
