Work Environment

This position involves home care therapy, allowing you to work directly in your patients' residences, which supports their comfort and convenience during rehabilitation. You will be closely engaging with patients in their familiar environments, facilitating a therapeutic experience that is as efficient as it is effective. Key Responsibilities

• Conduct comprehensive evaluations of patients to identify needs and develop tailored treatment plans, focusing on both orthopedic and neurologic conditions.
• Utilize advanced software to objectively measure patient outcomes and monitor progress throughout the rehabilitation process.
• Employ a range of therapeutic techniques during home visits, including manual therapy, exercise prescription, and functional training to restore mobility and enhance quality of life.
• Efficiently document therapy sessions, including evaluations, treatment plans, re-evaluations, and discharge summaries, ensuring compliance with all policies and regulations.
• Continuously assess data relevant to patient recovery and adapt treatment plans based on individual age-specific needs, ensuring a customized approach to care.
• Collaborate with interdisciplinary teams, including physicians and healthcare providers, to ensure comprehensive patient care and to discuss patient progress.

Patient Population / Client Focus

In this role, you will work with a diverse patient population that presents a range of orthopedic and neurologic conditions. Our clientele includes individuals recovering from surgeries, managing chronic pain, or undergoing rehabilitation due to injuries. Your skills will help restore mobility and improve function, focusing on enhancing each patient's overall well-being.

Qualifications

The ideal candidate must possess the following qualifications:

• A Bachelor's degree in Physical Therapy from an accredited program.
• Current BLS/HealthCare Provider certification.
• A valid Physical Therapist license in the State of Florida; a temporary permit will be acceptable for candidates awaiting full licensure.

Preferred Experience

While not mandatory, we prefer candidates with:

• Experience in home health settings or outpatient orthopedic care.

Compensation and Benefits
