Key Responsibilities\n

• Psychosocial Assessment: Conduct thorough psychosocial assessments for patients and families, addressing behavioral and emotional factors that can impact treatment outcomes. Ensure assessments are completed promptly, adhering to ethical and legal standards.

• Clinical Intervention: Provide supportive counseling to patients and families experiencing complex medical decisions, such as end-of-life care. Facilitate family conferences and help facilitate communication between families and healthcare providers.

• Case Management: Manage an ongoing caseload, effectively balancing high-risk admissions and referrals. Utilize your clinical judgment to identify urgent situations and collaborate with colleagues to develop appropriate care strategies.

• Documentation: Maintain accurate and up-to-date records of interventions, assessments, and patient encounters in compliance with organizational policies, including interdisciplinary care plans and progress notes.

• Quality Improvement: Participate in initiatives aimed at enhancing the quality of care within the clinic, contributing to strategy formulations and performance indicators for improvement.

• In-Service Training: Provide educational sessions for fellow staff members on psychosocial aspects of patient care, contributing your expertise in various forums such as Lunch and Learns.

• Supervision: Supervise graduate students and clinical associates, guiding their development and ensuring compliance with clinical standards.

Patient Population / Client Focus\n

This role will focus on providing social work services to a diverse patient population within our Community Health Clinic. Qualifications\n

• Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

This full-time position offers an annual salary range of USD 120,000 to 130,000 , commensurate with experience.
