Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients through interviews, observations, and medical record reviews to evaluate the emotional, social, and environmental factors impacting recovery.

• Provide therapeutic counseling and emotional support to patients and their families, addressing challenges related to adjustment and emotional well-being throughout the recovery process.

• Collaborate with rehabilitation specialists to develop and implement individualized treatment plans that take into account each patient’s unique psychosocial needs, goals, and circumstances.

• Assist patients in accessing vital community resources, including support groups, vocational rehabilitation services, and housing assistance, to enhance their recovery and reintegration.

• Educate patients and families about available community resources and post-rehabilitation services that facilitate continued growth and independence.

• Participate in discharge planning, ensuring a smooth transition for patients returning home or to alternate care settings by coordinating necessary services and supports.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
