Geriatric Nurse Practitioner: Primary Care & Telemedicine

PACE Southeast Michigan

PACE Southeast Michigan is hiring a Nurse Practitioner (NP) to deliver primary care to participants. Responsibilities include chronic disease management, coordination with interdisciplinary teams, and after-hours care as needed. The ideal candidate will have 3-5 years of RN experience in a clinical setting, preferably in geriatrics, and a valid Michigan NP License.
