Wellnest is seeking an ICMS Case Manager in Los Angeles to join a multidisciplinary team providing service planning and housing support for Transition Age Youth (18-25). The role emphasizes healing-centered, harm reduction, and housing first approaches. You will coordinate care, connect clients with housing, health, and education resources, and document progress in electronic systems. The ideal candidate will have a BA/BS in social work or related field, experience with homelessness or mental #J-18808-Ljbffr
