Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their mental health needs and develop personalized treatment plans.

• Maintain open lines of communication with supervisors, healthcare staff, and clients’ family members to provide updates on treatment progress and make collaborative decisions.

• Create and share a structured schedule that includes client appointments, home visits, and follow-up observations to track the advancement of treatment goals.

• Coordinate in-home visits as necessary, including chart reviews and interviews, to gather pertinent data and insights regarding client challenges.
\n
• Implement and monitor treatment plans, ensuring that therapeutic interventions are effective and aligned with client needs.
\n
• Engage in Interdisciplinary Team Meetings, contributing to discussions that enhance the care delivered to our patients.
\n
• Document client progress through monthly and quarterly progress notes, ensuring adherence to legal and ethical standards.
\n
• Organize and lead training sessions for team members to ensure proficiency in implementing treatment plans and documentation procedures.
\n
• Facilitate ongoing support through follow-up interviews and home visits, adjusting care as clients progress towards their goals.

• Act as a liaison between Aid Sphere Solutions and the broader mental health community, advocating for client needs and development goals.

• Contribute to the creation of training materials to optimize existing programs and improve service delivery.

Patient Population / Client Focus\n

This role offers the opportunity to work with a diverse range of clients, including adults and adolescents facing various mental health challenges. You will play a critical role in addressing issues such as anxiety, depression, trauma, substance abuse, and family dynamics. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Clinical Professional Counselor (LCPC) in the state of Illinois is mandatory.

• Minimum of two years of direct counseling experience, preferably in a clinical setting.

• Demonstrated specialization or experience in areas such as family therapy, couples counseling, substance misuse, or trauma-informed care is highly preferred.

• Strong understanding of legal and ethical practice in the mental health field.

• Excellent interpersonal and communication skills, with the ability to build rapport with clients and collaborate effectively within a healthcare team.

• The annual salary for this position ranges from $90,000 to $110,000, commensurate with experience and caseload.
