Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients’ emotional, social, and environmental circumstances in relation to their health challenges.

• Develop and implement individualized treatment plans using evidence-based social work interventions that incorporate input from patients and their families.

• Facilitate effective communication and collaboration among the care team, ensuring all members are informed about patients' progress and needs.

• Lead discharge planning efforts by coordinating with various healthcare professionals, payers, and community resources to ensure successful transitions to outpatient care.

• Provide therapeutic interventions, including crisis intervention, individual, group, and family therapy tailored to the unique needs of each patient.

• Teach and educate patients and families about coping strategies and support systems available to facilitate their adjustment to recovery.

• Participate in on-call rotations to address after-hour patient needs and emergencies effectively.

• Present at educational conferences, sharing knowledge about psychosocial aspects of oncology and addiction recovery.

• Engage in community outreach initiatives that aim to strengthen support systems for patients and cancer survivors through education and collaboration with local resources.

Qualifications\n

Compensation and Benefits\n
