Physical Therapist - Home Health Schedule Tuesday - Saturday, 8 AM to 5 PM Job Description A Physical Therapist is needed to provide one-on-one therapy care for patients in their home setting. The ideal candidate will assess a person’s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Responsibilities Assess patient conditions and develop individualized care plans. Collaborate with an interdisciplinary team to ensure a patient's needs are met. Perform standard therapy measurement techniques, including ROM, strength, and balance tests. Utilize a variety of therapeutic modalities to maximize care provided to clients. Required Experience / Certifications / Licensure Active Vermont Physical Therapist licensure. Home Health experience is a must, along with OASIS experience for case management.
