Pay package discussed upon application. What You'll Do

• Administer medications and treatments as ordered, accurately recording all care in the EHR/EMR system
• Monitor resident vital signs, blood glucose, and laboratory specimens
• Assist physicians during rounds, examinations, and treatments
• Update and review care plans in collaboration with the care team
• Report significant resident changes to RN Supervisors promptly
• Supervise Nursing Assistants to ensure quality resident care
• Support residents with participation in activities and daily routines
• Maintain a clean, safe, and organized environment
• Document incident reports, resident weights, and medication logs
• Assist with resident transfer procedures, including documentation and communication with families
• Perform additional duties as assigned by supervisors

What You Need

• Valid LPN license in Pennsylvania
• BLS certification required
• Previous experience in long-term care or nursing home setting preferred, but new grads are encouraged to apply
• Strong communication and teamwork skills
• Ability to pass health screenings and physical assessments
• Knowledge of infection control standards
