Hospice RN Case Manager - Care Team Leader
St. Croix Hospice
St. Croix Hospice is seeking an RN Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff. You will provide day-to-day patient care, manage treatments, administer medications, maintain records, and coordinate with the care team to meet changing patient needs.
