21HHS in Sacramento is seeking a Part-time Registered Nurse Case Manager to perform SOC admissions, follow-ups, and home visits throughout the Greater Sacramento Area. The schedule features every Saturday and Sunday full days, with potential for a 3-day work week, and all activities involve patient homes.
