Key Responsibilities\n

• Conduct thorough biopsychosocial assessments of clients and families, identifying unique social, emotional, and environmental challenges that must be addressed.

• Develop comprehensive individualized service plans with measurable goals and interventions tailored to the needs of each client, continually monitoring progress and adjusting plans as necessary.

• Provide compassionate individual and family counseling sessions focused on mental health needs, bereavement support, and coping strategies throughout the hospice process.

• Implement crisis intervention strategies, performing risk assessments and safety planning to ensure clients are protected and supported during emergencies.

• Collaborate with healthcare teams and other community resources to facilitate access to essential services and streamline care for clients.
\n
• Engage in critical community outreach and awareness initiatives to educate the public about hospice care and our agency’s services.

• Attend regular case conferences and contribute to team meetings to ensure consistent communication, strategy development, and shared learning across disciplines.

Patient Population / Client Focus\n

This role primarily focuses on individuals and families facing serious, often terminal health conditions. Clients may be dealing with emotional and psychological stressors surrounding their illness, end-of-life decisions, and the dynamics of anticipatory grief. Through active listening, empathetic support, and resource connection, you will play a crucial role in enhancing their overall well-being and dignity.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program is mandatory.

• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.

Compensation and Benefits\n
