Work Environment\n

This position offers the flexibility of remote work, allowing you to deliver care and support to seniors from the comfort of your own home while maintaining a focus on the client’s needs. As part of our interdisciplinary team, you will have opportunities to collaborate with healthcare professionals, nurses, and caregivers to coordinate high-quality care for our clients. Your work hours will primarily be Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed to accommodate the schedules and needs of the clients.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, focusing on emotional, social, and mental health factors that impact their well-being.

• Develop personalized care plans collaboratively with clients and their families, ensuring goals align with their preferences and healthcare needs.

• Provide individual and family counseling to support seniors during transitions, health challenges, and emotional difficulties.
\n
• Advocate for clients’ rights and access to community resources, facilitating connections to the support services they require.
\n
• Engage in regular interdisciplinary team meetings to share insights and collaboratively enhance care plans.

• Monitor clients’ living conditions through home visits, identifying any safety concerns and addressing their evolving needs.

• Facilitate communication among family members to promote understanding and resolve conflicts related to care decisions.

• Educate clients on available community resources, social services, and programs to foster social engagement and improve quality of life.

• Maintain accurate documentation of client interactions, assessments, and care plans in compliance with organizational standards and regulations.

• Stay current with industry practices, policies, and regulatory requirements relevant to social work and hospice care.

Patient Population / Client Focus\n

Your primary focus will be on the senior population receiving hospice care, where compassion and understanding are paramount. You will work closely with clients and their families, navigating the emotional landscape of end-of-life care, helping them cope with grief, loss, and the transitions associated with aging and illness.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in social work, preferably in a hospice, geriatric, or community setting.

• Extensive knowledge of aging-related issues, psychosocial principles, and community resources available for seniors.

• Exceptional assessment, communication, and problem-solving abilities to effectively address client needs.

• A deep sense of empathy and ability to build rapport with clients and their families.

• Strong collaborative skills to work successfully within interdisciplinary teams.

• Proficiency in electronic health record (EHR) systems and other relevant software applications.

Compensation and Benefits\n

We offer a competitive salary range of $60,000 to $100,000 per year, commensurate with experience and qualifications.
